Your First Visit

At Associated Retinal Consultants, our caring and compassionate physicians and staff are committed to providing you with the highest quality care.

On your first visit with our physicians, you will be thoroughly evaluated which will involve dilation of both eyes. Therefore, we recommend you plan on bringing a driver with you and also plan on being at your appointment for 1 ½ to 2 hours.

We recommend you complete the following forms in advance of your appointment if possible.

In addition, please bring the following to your appointment:

  • Current medication list
  • Picture I.D.
  • Insurance Card(s)
  • Sunglasses
  • A referral from you primary care physician, if your insurance requires this document (typically for HMO’s).

We are committed to your safety when visiting our office and have implemented a number of additional precautionary measures including additional cleaning/disinfecting procedures, social distancing practices and all staff/physicians are screened daily and are wearing PPE including masks.

When visiting our offices, the following precautionary measures will be required of our patients for your safety and the safety of our physicians and staff:

  • Screening for COVID-19 symptoms
  • Temperature check
  • All patients must wear masks
  • Guests of patients will not be allowed in the office except for a single legal guardian or a guest for patients requiring physical assistance in the office as determined by our personnel
